As always, if you do not have Direct Deposit, your check will be delivered to your work site; unless you request for it to be held at the

District Office.


All checks at the District Office will be put in the mail at 1:00 pm, unless a special request is made.


If you request to pick your check up, they will be available from

7:30 am - 4:30 pm

Employee Forms

Information change packet: For changes in marital status and/or beneficiary designation.

W-4 Forms - new options available


Current employees who complete a new Federal Tax Withholding W-4 form are now required to also complete the new State Tax Withholding W-4 form.   To change just State Tax Withholding employees will need to complete the new State W-4 form.  It is not necessary to change Federal Tax Withholding W-4 if just making a change to State Tax Withholding.  If you have already completed the Oregon W-4, you will only need to fill out a new one if you wish to make changes.


Insurance Opt Out Information and Forms


  • To determine your Opt-Out eligibility, please refer to: Collective Bargaining Agreement Article 13 (Classified Staff) or Article 6 (Licensed Staff)
  • If you qualify for early retirement coverage and plan to retire this plan year, do not opt out.
  • You will need to have your proof of other group coverage when you opt out on the OEBB website.
  • You will need to opt out of health coverage, and decline vision and dental. Keep the LTD and life coverage.. The district will continue to provide you with life insurance and LTD coverage (if eligible).

Retirees who do not qualify for any District Early Retirement Benefit;   (100% Self pay retirees will be transitioned directly to OEBB). Below is the ACH Debit form for OEBB, and letters giving additional information on transitioning to OEBB:

If you are retired and qualify for the District's Early Retirement Program, please download this form, fill out and return to Lisa Strickland in the business office if you would like to have your health insurance premium deducted automatically from your bank account.  (This is for retirees who qualify for the District's Early Retirement Program).
