Mail–Order Pharmacy Services
Mail–order is offered through Postal Prescription Services (PPS). If you would like to use your mail–order benefit, we encourage you to register with PPS as soon as possible by downloading a registration form at www.ppsrx.com, completing and mailing it to PPS or by calling PPS. PPS Customer Service: 800–552–6694.

The Employee Assistance Program (EAP) can help you solve personal and professional problems so you can regain balance in your life, at work and at home. The EAP provides counseling (3 free visits per new issue), 24/7 crisis support, and access to counselors online. You also have wellness and work-life balance services, including legal and financial help, will preparation, online tools and information. Confidential services are available at no cost to you and cover your dependents and all household members, related or not.
Retirees
Retirees who do not qualify for any District Early Retirement Benefit; (100% Self pay retirees will be transitioned directly to OEBB). Below is the ACH Debit form for OEBB, and letters giving additional information on transitioning to OEBB:

If you are retired and qualify for the District's Early Retirement Program, please download this form, fill out and return to Venice Anderson in the business office if you would like to have your health insurance premium deducted automatically from your bank account. (This is for retirees who qualify for the District's Early Retirement Program).

Insurance Opt-Out
Before You Start:
To determine your Opt-Out eligibility, please refer to: Collective Bargaining Agreement Article 13 (Classified Staff) or Article 6 (Licensed Staff)
If you qualify for early retirement coverage and plan to retire this plan year, do not opt out.
You will need to have your proof of other group coverage when you opt out on the OEBB website.
You will need to opt out of health coverage, and decline vision and dental. Keep the LTD and life coverage. The district will continue to provide you with life insurance and LTD coverage (if eligible).
